To change the last name on your account, please fill out the Name Change Request Form along with the Amended Distributor Application. These are all in one form and can be found in eVO under the Communications tab, Library. We will also need a copy of the legal document showing the change – Marriage License/Divorce Decree/etc… Once completed, forward all appropriate documentation together to compliance@lifevantage.com.
